Minor Work Permits

Glenbrook South High School issues Minor Work Permits on behalf of the Illinois Department of Labor. GBS does not have the discretion to alter or waive requirements that have been defined by the Illinois Department of Labor.

Work Permit Application Instructions

Please note: work permits can only be issued once all requirements have been met and all steps are completed. To request a work permit, the student must have a job offer lined up. Work permits are issued under the employer’s name and cannot be used for multiple businesses.

• Print and complete the Parent Statement
• Print the Employer Statement and ask the employer to complete it
• Gather the following documents
  • Physical or doctor note dated within one year of the work permit request
  • Colored scan/picture of birth certificate or passport
• Submit the worker’s documents through the online form. 
• Once the permit is ready for pick-up, the submitter will receive an email with pick-up instructions.
