It is the policy of the Board of Education of District 225 to provide homebound or hospital bound instruction at district expense for students who are unable to attend school for two weeks or longer for reasons of ill health. In order to implement this policy, Glenbrook South has adopted a procedure to facilitate the student's learning during his or her absence.


As soon as a parent or guardian is aware their student will be out of school for more than two weeks, the Homebound Coordinator should be contacted. A physician must complete the Medical Certification Form and Release of Information. This form must include a diagnosis and estimated length of absence. Homebound services cannot be initiated until these forms are returned. 


The process begins when the completed Medical Certification Form and Release of Information is received by the Homebound Coordinator. The Homebound Coordinator will contact you to review the Homebound process and answer any additional questions you may have.
