What is Transition?
The term "transition services" means a coordinated set of activities for a child with a disability that—
(A) is designed to be within a results-oriented process, that is focused on improving the academic and functional achievement of the child with a disability to facilitate the child's movement from school to post-school activities, including post-secondary education, vocational education, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation; (B) is based on the individual child's needs, taking into account the child's strengths, preferences, and interests; and (C) includes instruction, related services, community experiences, the development of employment and other post-school adult living objectives, and, when appropriate, acquisition of daily living skills and functional vocational evaluation.

Glenbrook South offers job coaching to students that need support in the community. Our job coach can also help you find volunteer and paid employment opportunities.
Glenbrook South also offers two job related classes: Work Study and Careers class.

Resources For Education
Staff at Glenbrook South offers individual educational planning for students. Meet with the Transition Specialist to discuss a variety of post secondary options that are available for students with disabilities, including college and alternate post-secondary options.
Resources For Independent Living
Glenbrook South offers multiple opportunities for students to build skills in the area of independent living. Classes such as Independent Living, Consumer and Drivers Education help students to build necessary skills for independence in the community. Staff also work to connect students and families to agencies and community resources for long term support in the community.
Resources For Recreation
Staff at Glenbrook South believe in full life planning for students. Staff will help to connect students to current and future opportunities for recreation.
