Sealed transcripts for Driver’s Ed must be signed for by the student in the Registrar’s office. Unofficial transcripts for good driver discounts may be obtained from the student’s counselor or the Registrar. GBS GRADUATES, GBE GRADUATES AND FORMER STUDENTS: Transcripts for GBS alumni are processed through Parchment, Glenbrook South’s official agent for sending alumni transcripts. Please go to www.parchment.com to order your transcript. Phone requests cannot be fulfilled. Health Record Requests CURRENT GBS STUDENTS Current students may request copies of their medical records by contacting the school nurse at 847.486.4498. GBS GRADUATES OR FORMER STUDENTS Health Records for all GBS graduates or former students must be made in writing – telephone requests cannot be fulfilled . Please complete the fillable Glenbrook South Alumni Health Record Request Form and send completed form and a photocopy of your State Driver's License or State ID to the address below (US Mail or email). This request may only be originated by the alumni requesting the health records, unless he or she is under the age of 18 . Visit the District Administration Building at 3801 W. Lake Avenue, Glenview to obtain and complete residency paperwork. Please review the Required Residency Documentation list prior to visiting the District Administration Building, to ensure you have all of the information necessary. Complete the Online New Student Application. During the school meeting, please be sure to bring the following information if applicable: Student's birth certificate or Family Register. Foreign documents must have translation. Illinois Transfer Form for those students transferring from another Illinois public school. Withdrawal papers and most current grades, along with unofficial transcript, if transferring from another high school. Incoming freshman must provide a copy of their test scores and/or placement recommendations. A copy of most current IEP or 504 Plan is required, if the student receives special services.
